Acupressure for Lower Back Pain: 3 Points and How to Press Them

Introduction
Most sore backs on most days are not broken in any dramatic way. They are tired from sitting, tight from lifting something awkwardly, or simply over-asked — and they complain the way tired things do. Chinese tradition maps three points for that everyday ache: one on the back itself, one behind the knee, one deep in the hip. Together they make a routine you can do in ten minutes with no equipment and no second person.
The framing matters more than the points. Back pain is also the symptom with some of medicine's scariest rare causes behind it, so this article spends as much space on "when not to do this" as on technique. Acupressure here is comfort for an ordinary muscular ache — not treatment for sciatica, not a substitute for imaging, and never a DIY answer to a red flag.
Point 1: BL23 (Shenshu, 肾俞) — beside the spine
Shenshu means "Kidney's Hollow" — the point sits at the same level as the kidneys, and its traditional job is the lower back's daily wear.
Where it is:
- Put your hands on your waist with thumbs pointing back, or ask someone to help.
- Find the narrowest part of your waist; BL23 sits roughly there, about two finger-widths out from the spine, at the level of the top of the hip bone's back edge.
- Pressing often finds a deep, satisfying soreness — the "yes, there" spot.
How to press: Standing or lying, knuckles or thumbs, steady pressure or small circles for 1–2 minutes each side. A tennis ball rolled between back and wall does the same job hands-free.
Point 2: BL40 (Weizhong, 委中) — behind the knee
A classical saying assigns the back's complaints to this hollow: when the back complains, look to the back of the knee.
Where it is: The soft hollow at the centre of the crease behind the knee, midline.
How to press: Seated with the leg slightly bent, or lying face down. Press gently — this hollow is more delicate than the back points — for 1 minute per side, a few rounds. The feeling should be a mild spreading ache, nothing sharp.
Point 3: GB30 (Huantiao, 环跳) — deep in the hip
Huantiao is the deep gluteal point on the sciatic nerve's path — traditionally paired with lower back stiffness that pulls into the hip.
Where it is: On the side of the hip, about one-third of the way along a line from the sacrum toward the hip bone's prominent point. Practically: the tender deep spot you find pressing firmly into the upper-outer buttock area while standing on the other leg — with the important exception below.
How to press: A firm ball against a wall, gentle but sustained, up to 2 minutes per side. If the spot radiates electricity or tingling down the leg — that is nerve territory, not a comfort ache — stop pressing it entirely.
What Actually Moves the Needle for a Tired Back
Honest hierarchy, both in tradition and in modern guidance:
- Movement. Ten minutes of walking does more for an ordinary aching back than any pressing. The tradition agrees — flow over stagnation.
- Heat. A warm pack or hot-water bottle on the sore band, the same instinct a foot-soak household applies elsewhere (our evening wind-down pages live in that family).
- The points. Useful as the third layer — a way to settle in the evening, fall asleep with less complaint, and feel a bit more limber the next morning.
None of the three fixes a disc, releases a pinched nerve, or addresses the reason the back keeps complaining. The fix for a desk-shaped back is a less desk-shaped day.
The Red Flags — Read This Part Twice
Back pain is ordinary most of the time and urgent a tiny fraction of the time. The urgent fraction is not pressable:
- Loss of bowel or bladder control, or numbness in the groin/saddle area — possible cauda equina syndrome, emergency room now
- Sudden severe pain after a fall or accident
- Progressive weakness, numbness, or tingling in a leg — nerve involvement needs assessment, not acupressure
- Fever alongside back pain — possible kidney or spinal infection
- Back pain with unexplained weight loss, or night pain that wakes you regardless of position — worth a doctor's attention promptly
- Pain that is clearly worsening week over week despite sensible self-care
None of these mean the three points caused harm; they mean the situation outran comfort care before it started.
A Simple Evening Routine
Warm pack on the back for 10 minutes → BL23 with knuckles, 2 minutes → BL40, 1 minute per side → skip GB30 unless the hip genuinely asks for it → slow walk around the house → bed. Ten minutes, no strain, and repeatable. If the same ache returns every week, the answer is a better chair, a lifted screen, and stronger legs — the points just make the wait more comfortable.
